If Telos feels right for your family, let us help you through the enrollment process. You can contact our admissions staff Kristin at 801-380-7980 (firstname.lastname@example.org) or Jaren Hamel at 480-313-9907 (email@example.com).
These three questions must be answered to access the online application and enrollment documents:
The average length of the Telos program is 10-11 months. Are you able to commit to a process that may require this amount of time?
Telos students struggle with issues like depression, anxiety, self-harm, learning differences, executive function deficits, and substance abuse. Does your son meet this criteria?
Telos' daily rate (after the underinsured scholarship) is $415, and is typically private pay. Is your family able to make a financial commitment at this level?
We do not treat students with a criminal history, a disposition toward aggression, issues with sexual perpetration, or those who are actively psychotic. Given this information, does your son meet our admissions criteria?
If you believe Telos is a good option for your family fill out this form and a representative will be in contact with you shortly.
